Ship time request forms can also be obtained upon request from the Marine Operations Center, Pacific (MOP) in Seattle, WA, or the Marine Operations Center, Atlantic (MOA) in Norfolk, VA, or NOAA Marine and Aviation Operations (NMAO) in Silver Spring, MD.

Instructions for Ship Time Request Form (NOAA Form 77-65)

The purpose of NOAA Form 77-65 is to provide NC and the the Fleet Allocation Council (FAC) with the information necessary to determine which ship(s) would be most likely to meet the requirements of a specific project.

NOAA Form 77-65 is to be submitted by program offices with the initial request for ship time. All available information is to be provided at that time. As additional information becomes available, or as conditionas warrant, another form shall be submitted. Forms specifying the change, shall be submitted to keep NC and the FAC advised of all pertinent facts.

Itemized instructions are as follows:

Items 1, 2 3 Self explanatory
Item 4 Indicate whether this is original request or an update. If an update, indicate the consecutive number.
Item 5 If an update, indicate dateof original request.
Item 6 Indicate name of ship or type of ship that you believe would be most desirable as platform for this particular project.
Item 7 Name of individual preparing this request.
Item 8 Indicate name of Chief Scientist, Chief Experimenter, et al.
Item 9 Indicate name and phone number of person to be contacted concerning details of this project if it is to be someone other than the Chief Scientist.
Item 10 Include brief description of the project, it's purpose and expected contribution to a broader program such as a national or international effort.
Item 11 Self explanatory
Item 12 Show both the maximum number of days desired and the minimum days in which meaningful work could be accomplished. Include transit time between the work area and home port or mid-project port calls.
Item 13 Indicate the season of the year that would be preferred for accomplishment of the project and state whether it is mandatory or if there would be acceptable alternatives.
Item 14 Indicate whether this project will require the dedicated use of the ship or can be a piggyback-type project with a minimum of interference to the principle users.
Item 15 Indicate optimum number of program personnel that would be desirable to have aboard and the minimum number with which the program could be carried out. Also, indicate the number of personnel of each category that would be available for watchstanding duties.
Item 16 If this project will require dedicated ship time, indicate whether or not there will be time and, if so, how much time available for piggyback projects or if piggyback projects could be accomodated on a non-interference basis.
Item 17 Specify the necessary capabilities which will be required of the ship to which this project will be assigned. Be sure to indicate whether or not personnel support will be required for the various items of equipment.
Item 18 Indicate minimum acceptable position accuracy in meters.
Item 19 Indicate type and extent of data processing required of ship's personnel. Also, indicate type of output required from the automated data processing system and any other output required.
Item 20 List all equipment to be brought aboard the ship by the program and provide the necessary specification. Also, include ship personnel support required.
Item 21 Indicate staging time required to install the aforementioned equipment.
Item 22 Should be approved at level with authority to establish overall program objectives. Must be approved by both originating office and program manager.
